Overview

The LPN Care Coordinator plays a critical clinical and operational leadership role in ensuring the delivery of high-quality, personalized care to clients. This position leverages nursing expertise to supervise caregivers, oversee compliance and training, conduct clinical assessments, and maintain strong client relationships. Serving as the clinical link between the caregiving team and leadership, the LPN Care Coordinator upholds The Perfect Companion’s commitment to excellence, dependability, and compassionate service.

Core Responsibilities1. Clinical Oversight & Supervision of Caregivers

  • Provide daily clinical and professional oversight to the caregiving team, ensuring care is delivered safely and in alignment with each client’s plan of care.
  • Act as a liaison between caregivers and leadership, ensuring prompt resolution of issues and maintaining clear, professional communication.
  • Conduct in-home supervision visits that include direct observation of care techniques, adherence to care plans, and infection control practices.
  • Serve as the on-call clinical lead for designated days each week, managing after-hours caregiver or client needs in a timely and professional manner.
  • Ensure caregivers follow all policies, procedures, and clinical standards; implement and document corrective actions as needed.
  • Personally provide hands-on care only as a last resort, maintaining a primary focus on oversight and coordination.

2. Orientation, Training & Clinical Education

  • Lead new caregiver orientation, ensuring clear understanding of company policies, procedures, and clinical expectations.
  • Oversee the completion of state-mandated training, competency checks, and skill validations.
  • Provide quarterly training or education sessions on clinical topics such as medication safety, vital sign monitoring, infection control, and emergency response.

3. Client Assessments, Care Planning & Quality Assurance

  • Conduct all initial and follow-up nursing assessments in accordance with state regulations and company protocols.
  • Develop and update individualized care plans, incorporating client medical history, current needs, and physician directives.
  • Maintain strong relationships with clients and families through regular communication, proactive adjustments to care, and routine in-home visits.
  • Manage quality assurance processes, including incident reporting, satisfaction surveys, and care plan audits.

Qualifications

  • Current Tennessee LPN license in good standing.
  • 2+ years of experience in nursing, care coordination, case management, or supervisory roles in home care, healthcare, or senior services.
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
  • Proficiency with scheduling systems and Microsoft Office.
  • Excellent interpersonal communication, client relations, and conflict resolution skills.
  • Commitment to professionalism, discretion, and the highest standards of care.

Preferred Qualifications

  • Experience in caregiver recruitment, training, and performance evaluation.
  • Knowledge of Tennessee home care regulations and nursing standards.
  • Background in senior living, concierge services, or private pay home care.
