Job Title: Case Manager

Our services include psychiatric evaluations, medication management, and therapy for diverse communities.
Position Summary
The Case Manager / Care Coordinator plays a vital role in ensuring patients receive seamless, holistic care by bridging clinical, administrative, and community support. This role combines care coordination, case management, and front office engagement to help patients navigate behavioral health treatment, overcome barriers, and achieve wellness.

Key Responsibilities:
Patient Support & Clinical Coordination
- Greet and check in patients; enter and update information in the EMR.
- Verify insurance and collect co-pays when applicable.
- Conduct and record vitals (BP, weight, temperature, drug testing) as needed.
- Schedule appointments and assist with medication coordination.
- Respond to patient calls and portal messages promptly and respectfully.
Case Management & Resource Navigation
- Complete needs assessments with a focus on social determinants of health.
- Develop and update individualized care plans in collaboration with providers.
- Connect patients to resources such as housing, SNAP, Medicaid, transportation, and legal support.
- Advocate for patients facing system-related barriers.
- Assist in discharge planning and transitions from hospitals, inpatient, or detox programs.
Outreach, Referral & Re-Engagement
- Make and track warm hand-offs to external partners.
- Conduct follow-up calls to monitor outcomes and satisfaction.
- Build and maintain relationships with community referral sources (e.g., DHS, shelters, schools).
- Re-engage patients in crisis or those who have disengaged from care.
Documentation & Compliance
- Maintain timely, accurate, and Medicaid-compliant documentation.
- Log outreach, resource use, and patient outcomes per internal and regulatory standards.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Human Services, Public Health, or related field required.
- Experience with Electronic Medical Records (EMR) preferred.
- Knowledge of Medicaid, HIPAA, trauma-informed care, and social service systems.
- Strong communication, time management, and problem-solving skills.
Work Environment & Schedule
- This is an in-person role based at our Englewood, CO location.
- Our Office hours are Monday–Friday, 8:00 AM – 6:00 PM, and Saturdays 10:00 AM – 3:00 PM.
- Flexible scheduling available within those hours (ideally 32–40 hours/week).
Examples: M–F 8:00 AM–4:00 PM, or M–Th 8:00 AM–6:00 PM. We are happy to accommodate your needs.
