The Social Worker works as part of the multidisciplinary Family Connects NJ (FCNJ) team, attends team meetings, contributes to case conference, documents in FCNJ HealthCloud. Provides services to individuals who are at risk for postpartum depression, including triage and support with connections to care. Facilitates regional support groups for FCNJ families.
Essential Functions:
Contribute to The Cooperative’s commitment to equity and continuous quality improvement that aligns with the agency’s mission, goals, and advancement. * Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation. * Routinely disseminate relevant information, presentations, training, and learning opportunities for internal program and agency staff. * When applicable, will participate in relevant staff development opportunities (trainings, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings. * At selected clinical sites, organizes perinatal support groups and assists the Program Coordinator in establishing the model of support that can be sustainable in each community; conducts, leads, and facilitates psycho-educational support groups for individuals with or at risk for PMD. * Provides triage and follow up so that the new mother gets the appropriate level of treatment in a timely fashion. * Provides referral to appropriate counseling services to individuals and families as indicated * Acts as a liaison between client, clinic staff and community-based programs in securing required services to meet continuing needs of client * Coordinates resource file of community, social, health, and welfare agencies involved in perinatal mood disorder treatment programs. * Performs other duties as assigned.
Requirements
- Master’s degree in social work; LSW preferred
- Two years’ experience in support group facilitation, triage process and specialized training involving perinatal mood disorders
- Two years’ clinical experience in working with childbearing families in an outreach or perinatal setting
- Ability to work independently and in groups with good leadership, oral and written communication skills
- Valid driver’s license, current insurance policy and car
Physical Requirements:
- Must be able to see to read documents and hear well enough to communicate with others
- Must be able to write, type on a computer, and use a telephone
- Must be able to operate a motor vehicle
- Must be able to lift up to 10 pounds
- Additional requirements to be determined with input from Director and HR

Work Environment:
Travels regularly to sites where traffic, weather, and other environmental conditions may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions
Candidate is expected to complete the necessary training to receive Perinatal Mental Health Certification (PMH-C). The cost of training and exam will be covered as part of compensation.
