Minimum Qualifications:
- Bachelor of Social Work from an Accredited Social Work Program or equivalent education and experience.
- Basic computer skills, including Microsoft Office Products.
Key Responsibilities:
- Communicate with SCI MAT Social Worker regarding reentrants who overdose in the community and return for detox to determine eligibility for MAT.
- Assist with MAT referrals in the community, including setting up and scheduling appointments for medication and treatment.
- Ensure that the needs, benefits, and services of assigned individuals are met according to their treatment plans.
- Track and update databases for individuals released on MAT from SCI.
- Attend weekly Vivitrol Conference Calls and other necessary meetings.
- Document all contacts in appropriate systems (Captor, Sapphire, etc.).
- Attend all required trainings.
- Meet with reentrants with positive urine drug screens to assess and refer to treatment.
- Complete required GPRA assessments.
- Meet with reentrants at SCI, BCC, Parole Offices, and/or Treatment providers, as necessary.
- Communicate treatment plans with Center Staff, SCI Staff, and Parole Staff.
- Adhere to DOC’s MAT/AOD Treatment framework, policies, and procedures.
- Provide social work, counseling, and case management services to enhance social functioning and adjustment.
- Perform in accordance with applicable rules, regulations, policies, and procedures under supervision.
- Assess offenders’ psychosocial strengths, weaknesses, and needs, and determine eligibility for services.
- Counsel individuals using various treatment modalities.
- Discuss psychosocial assessments and family histories in multidisciplinary team meetings.
Required Skills:
- Knowledge of professional social work and casework principles, practices, and methods.
- Understanding of individual and group behavior.
- Proficiency in counseling techniques and interventions.
- Awareness of commonly occurring physical and mental illnesses and their implications.
- Familiarity with social, economic, health, habilitative, and rehabilitative problems and resources.
- Understanding of HIPAA and state confidentiality laws.
- Ability to interpret rules, regulations, policies, and procedures.
- Strong interpersonal and communication skills, both written and oral
