How you will make an impact:
The Care Coordinator plays a vital role in advancing the Ministry, Mission, and Core Values of The Lutheran Home and Lutheran Life Communities through professional, ethical practices and exceptional hospitality. This position is responsible for managing intake calls and following up on all incoming leads, while actively promoting MySolutions Home Care services throughout the Lutheran Life Communities, particularly at the LH campus. The Care Coordinator routinely visits new arrivals in MyRehab shortly after admission and again prior to discharge, helping ensure a smooth transition of care.
By assessing client needs, developing individualized service plans, and overseeing the delivery of MySolutions home care and handyman services, the Care Coordinator ensures that each client receives high-quality, personalized support. Through regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessments, they build and maintain strong relationships with clients, families, and caregivers.
This role takes a holistic approach by integrating direct and indirect services, connecting clients to community resources, and advocating on their behalf to exceed satisfaction and support overall well-being. The Care Coordinator also assists with caregiver scheduling, onboarding and training new staff, and coordinating quarterly and annual training sessions.
Schedule Varies: On-call as needed; typically scheduled Monday through Friday from 8:00 AM to 4:00 PM, with occasional evening and weekend availability required.
What you will need:
- College degree preferred;
- Sales, Healthcare or Social Work background a plus.
- Supervisory experience a plus.
- Must be able to read, comprehend and write legibly in English.
- Must be able to comprehend and perform oral and written instructions and procedures.
- Must be able to calculate math figures and amounts.
