About Job
Intake Coordinator's primary function is to facilitate the intake referral process and coordinate care with the interdisciplinary team and the referral source.
- Verify Insurance benefits to confirm initial insurance coverage.
- Request prior authorization for all patients insurance.
- Enters patient information regarding authorizations/coverage approval.
- Follow up coordination of all referrals and maintain detailed comprehensive documentation in EMR.
- Follow up customer service calls for referrals and patients.
- Provide excellent and effective communication to patient/family team members and other health care professionals and maintains confidentiality.
- Demonstrate commitment, professional growth and competency.
- Complete all other duties as assigned in a timely manner.
- Referral Intake Coordinator must communicate all aspects and processes of new admissions in a timely manner.
- Assist in answering phone calls.
Job qualifications:
- High school diploma or equivalent
- At least 1 year of Home Health referral/insurance verification/authorization experience preferred.
- Must be able to read and write in English and follow instruction
- Employee should possess reasonable knowledge in computer systems
- Able to communicate well both verbally and written.
Job Type: Full-time
Pay: $17.00 - $18.00 per hour
