About Job

Intake Coordinator's primary function is to facilitate the intake referral process and coordinate care with the interdisciplinary team and the referral source.

  • Verify Insurance benefits to confirm initial insurance coverage.
  • Request prior authorization for all patients insurance.
  • Enters patient information regarding authorizations/coverage approval.
  • Follow up coordination of all referrals and maintain detailed comprehensive documentation in EMR.
  • Follow up customer service calls for referrals and patients.
  • Provide excellent and effective communication to patient/family team members and other health care professionals and maintains confidentiality.
  • Demonstrate commitment, professional growth and competency.
  • Complete all other duties as assigned in a timely manner.
  • Referral Intake Coordinator must communicate all aspects and processes of new admissions in a timely manner.
  • Assist in answering phone calls.

Job qualifications:

  • High school diploma or equivalent
  • At least 1 year of Home Health referral/insurance verification/authorization experience preferred.
  • Must be able to read and write in English and follow instruction
  • Employee should possess reasonable knowledge in computer systems
  • Able to communicate well both verbally and written.

Job Type: Full-time

Pay: $17.00 - $18.00 per hour
