- Lead and mentor a team of care managers to ensure the delivery of high-quality, patient-centered care.
- Develop and implement policies, procedures, and workflows to enhance clinic efficiency and care coordination.
- Oversee patient assessments, care planning, and interventions to maintain compliance with healthcare standards.
- Collaborate with multidisciplinary teams to optimize patient outcomes and streamline service delivery.
- Monitor performance metrics, analyze data, and prepare reports to identify improvement opportunities and inform strategic decisions.
- Ensure regulatory compliance and uphold organizational standards across all care management activities.
- Facilitate team meetings and training sessions to promote continuous learning and best practices.
- Current state Registered Nurse or LPN License as required by location.
- Registered Nurse experience - Two years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past three years.
- LPN experience - a minimum of Five years’ primary care practice experience with adult medicine and pediatric patients in a primary care/family practice setting.
- Care management experience preferred.
