Why does the job exist?
This position ensures seamless coordination of patient discharges by collaborating with clinical teams, patients, families, and community partners to develop safe, comprehensive, and person-centered discharge plans. The discharge planner plays a critical role in reducing readmissions, ensuring continuity of care, and connecting patients to the resources and services they need for long term recovery, and stability in the community.
How does it get done?
- Develop, coordinate, and implement individualized discharge plans for patients transitioning from inpatient care to community-based settings.
- Conduct assessments to identify patient needs, including housing, outpatient treatment, and support services.
- Collaborate with clinical teams, social workers, and external agencies to ensure smooth transitions of care.
- Provide case management services, including intake assessments, referrals, and follow-up coordination with service providers.
- Maintain accurate and timely documentation including discharge summaries and progress notes.
- Educate patients and families on post-discharge resources and treatment plans.
- Advocate for patients by coordinating with local behavioral health providers, housing programs, and community support organizations.

The role serves patients receiving inpatient behavioral health treatment at NMBHI, along with their families, guardians, and care teams. it also supports the hospitals clinical staff and community service providers by ensuring safe, effective, and well-coordinated discharges.

Minimum Qualification
High School diploma or Equivalency and two (2) years of experience in a substance abuse and or behavioral disorder counseling environment. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.

Employment ls subject to nationwide criminal background checks and is conditional pending results.
2. CRIMINAL RECORDS BACKGROUND CHECK-Must successfully complete post offer pre-employment criminal records background check. Employment is conditional pending the results of criminal records background checks. Must possess and maintain a valid driver's license. Successful completion of the Health Insurance Portability and Accountability Act (HIPAA).
Working Conditions
Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage. Effective 01/01/2015, the New Mexico Behavioral Health Institute became a 100% Tobacco Free/E-Cigarette Free Campus. As such, no tobacco, tobacco products or E-Cigarette products are permitted on any part of the campus.
