About Job

Description: The Licensed Clinical Social Worker (LCSW) will be responsible for review of psychological and social development programs pertaining to the residents of the children program at Destiny’s Group Home, Inc. LCSW will partake in the Admission, Review and Discharge team (ARD) with respect to intake referrals and proposed discharge of youth in services. LCSW will assist youth residents in handling challenging life situations, providing them with available resources and counseling them in the process. LCSW will also develop and monitor implementation of behavior intervention/behavior support plans (BIP/BSP) for youth with behaviors of concern to their wellbeing. The LCSW will collaborate with program administration to improve quality of care and support operation.

Delegating Nurse

Education and/or experience:

  • College Diploma
  • Master’s degree in social work
  • Received clinical training in behavioral principles and analysis
  • Previous experience preferred
  • Competent computer skills including MS Office or equivalent
  • Electronic Correspondence including use of e-mails, group messaging and data collection
  • Numeric and literacy skills
  • CPR/First Aid training

General Tasks and Responsibilities:

  • Site visits and engagement with youth population.
  • Counseling of youth residents and provision of recommendations to Residential Coordinators, Quality Enhancement, Program Director and Delegating Nurse.
  • Presentation and acquisition of resources.
  • Type documents, reports and correspondence.
  • Create and implement long term goals for youth residents of agency.
  • Review and provide feedback on intake referrals.
  • Attend children program administrative meetings.
  • Participate in youth residents’ Individual Support Plan (ISP) meetings.
  • Conduct detailed assessment of prospective program participants.
  • Complete Suicide Risk assessments.
  • Communicate and coordinate with other administrative members.
  • Assist with agency event planning and implementation.
  • Develop Behavior Plans and supervise implementation/progression.
  • Update behavior plans when applicable and/or required.
  • Train direct support staff on behavior plans and tools.
  • Analyze areas for expansion of services.
  • Ongoing communication with Delegating Nurse and Program Director regarding recommendations.
  • Maintain licensure and certification status as a LCSW.
