Location: 6701 Baymeadow Drive, Suite B, Glen Burnie, MD 21060 Hours: 40 hours per week Annual Budgeted Amount: $40,356 Benefits: Participation in the County’s group health plan including prescription, dental, and optical, paid holidays and sick leave. Optional deferred compensation program at employee's own expense.
Job Description
Nature of Work:
Under the supervision of the Maternal and Child Health Program Manager, this position will lead, support, monitor, and coordinate strategic plan activities related to countywide intervention efforts to strengthen perinatal care coordination. Particular emphasis will be placed on perinatal care coordination infrastructure building activities; strengthening referrals systems and networks that have identified high-risk pregnant birthing individuals and high-risk postpartum individuals to assure that the Healthy Start program is receiving these referrals to initiate contact with these individuals; and conducting an ongoing analysis of the current triage process, follow-up, and care coordination to make recommendations to improve perinatal care coordination.
Education
Minimum Qualifications
Bachelor’s degree in Public Administration, Healthcare Administration, Nursing, Behavioral Health, Social Work, Pschology, or other related field from an accredited college or university.
Experience
Bachelor's degree with two (2) years of related professional experience or Master's degree in one of the fields above and one (1) year of experience.
Desired Or Preferred Qualifications
Strong time managment and organizational skills; ability to work on a team and independently; excellent written and oral communication skills; advanced understanding of the public health system; proficient in Microsoft word and Excel.
