Job Summary
The Geriatric Care Manager plays a crucial role in providing comprehensive care and support to older adults and their families. This position involves coordinating medical services, facilitating communication with care teams, and ensuring that individuals are living with vitality and happiness. The ideal candidate will possess a strong background in communication, strong interpersonal skills and complex work environments. Our practice is the intersection of medicine, elder law issues, complex family dynamics and vibrant adults. This position is for an experienced professional, this is not for entry level applicants.
Responsibilities
- Assess the medical, emotional, and social needs of older adults to develop personalized care plans. Ability to document accurate care notes.
- Reliably coordinate with healthcare providers, including hospitals, Assisted Living facilities, Skilled Nursing and individuals aging at home.
- Monitor client progress and adjust care plans as necessary to meet changing needs.
- Provide education and support to clients and their families regarding health management and available resources.
- Collaborate with multidisciplinary teams to optimize individual outcomes through empathic listening, advanced problem solving and resource coordination.
- Advocate for older adults rights and preferences in all aspects of their care.
- Transport clients to appointments.
- Exercise judgement and communicate effectively in a dynamic, ever-changing environment.
Skills
- Strong background in social work especially geriatrics and or medical social work. LCSW or soon eligible for LCSW strongly preferred.
- Proficient in planning processes and care management.
- Excellent communication skills for effective interaction with individuals, families, legal teams, healthcare professionals and care partners.
- Ability to perform thorough clients assessments and develop comprehensive care plans.
- Compassionate approach towards client care with a focus on enhancing quality of life for elderly individuals.
- Flexibility to address urgent or unexpected needs.
- Ability to work in a dynamic environment that includes seeing individuals in their homes or facilities in the Denver Metro area.

Job Types: Full-time, Part-time
Pay: $65,000.00 - $70,000.00 per year
