Job Overview
Passionate Heart Hospice is seeking a Part-Time/ Per diem Hospice Social Worker to provide compassionate emotional, social, and practical support to patients and families during the end-of-life journey. This role will serve patients in Orange County, Los Angeles, and Riverside locations. The Hospice Social Worker will assess psychosocial needs, offer counseling services, and ensure patients and families receive dignified, personalized care during this important time.
Key Responsibilities:
- Patient and Family Support: Provide emotional and social support to patients and their families, addressing grief, loss, and other end-of-life concerns.
- Psychosocial Assessments: Evaluate patients' and families' emotional, social, and practical needs to identify goals and strengths.
- Care Planning: Collaborate with the hospice interdisciplinary team to create and implement personalized care plans.
- Counseling: Offer individual, family, and group counseling, focusing on stress reduction, coping strategies, and grief management.
- Crisis Intervention: Respond to urgent situations with immediate support and appropriate referrals.
- Resource Coordination: Help families access community resources, financial assistance, respite care, and social services.
- Education and Advocacy: Guide families on hospice care, advance directives, and end-of-life planning while advocating for patients' needs.
- Documentation: Maintain accurate and timely records of assessments, care plans, and interventions in compliance with hospice policies.
- Interdisciplinary Collaboration: Participate in team meetings to provide psychosocial insights and ensure holistic care.
- Cultural Sensitivity: Deliver culturally competent care, honoring diverse values, beliefs, and backgrounds.
Qualifications:
- Education: Master’s Degree in Social Work (MSW) from an accredited program.
- Licensure: Active state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent, as required by California law.
- Experience: 1–2 years of experience in hospice, palliative care, or healthcare social work is highly desirable.
- Skills: Knowledge of end-of-life care, grief counseling, and crisis intervention techniques.
- Strong communication and interpersonal skills for effective collaboration with patients, families, and interdisciplinary teams.
- Familiarity with community resources and support services.
- Excellent time management and organizational abilities to handle multiple priorities.
- Additional Requirements: Empathetic, compassionate, and patient-centered approach to care.
- Ability to work independently and as part of a collaborative team.

Initial Assessment: $100 PER VISIT - mileage included
- Locations: Orange County, Los Angeles, and Riverside areas.
