Transcend Hospice & Palliative Care is seeking a compassionate and skilled Hospice Social Worker (MSW) to join our interdisciplinary team. In this meaningful role, you will provide emotional, psychosocial, and resource support to patients and their families, helping them navigate the end-of-life journey with dignity and comfort. The Hospice Social Worker must be able to travel to our service areas of Morris County.

JOB RESPONSIBILITIES:


  • Responsible for conducting comprehensive psychosocial assessments to evaluate the needs of patients and families.

  • Provide emotional support and counseling to patients and their loved ones.

  • Assist in advance care planning, including discussing goals of care, end-of-life decisions, and advance directives.

  • Help families navigate grief, loss, and anticipatory bereavement.

  • Act as an advocate for patients and families, ensuring their needs and wishes are respected.

  • Educate families on available hospice services, palliative care options, and community resources.

  • Coordinate access to financial, legal, and social support programs as needed.

  • Work closely with the interdisciplinary team (nurses, chaplains, physicians, and home health aides) to ensure a seamless care experience.

  • Participate in bereavement services, providing counseling and referrals to families after a patient’s passing.

  • Lead or assist in grief support groups, memorial services, or bereavement outreach programs.

  • Maintain accurate and timely documentation of patient assessments, care plans, and interventions in compliance with hospice regulations.

  • Adhere to Medicare, Medicaid, and state regulatory guidelines for hospice social work services.

  • Attend and participate in in-services, staff meetings, and patient care conferences.

  • Counseling family members to help them know what to expect during the end-of-life process.

  • Adhering to infection control, standards of care and patient privacy standards.

  • Ensure that documentation is done promptly in the EMR.


QUALIFICATIONS:


  • Master’s degree in social work (MSW) from an accredited program

  • Licensed Social Worker (LMSW, LCSW, or equivalent, depending on state requirements).

  • Prior home care and/or hospice experience is strongly preferred but not required.

  • Strong communication, counseling, and crisis intervention skills.

  • Ability to work with diverse patient populations and handle sensitive end-of-life discussions.

  • Possesses strong problem-solving, decision-making and leadership skills in both typical and exceptional situations.

  • Valid driver’s license, personal vehicle for on-the-job transportation, and evidence of insurance coverage as required by Transcend Hospice & Palliative Care policy required

: Monday- Friday 8:30 a.m.- 4:30 p.m.
