About Job

About the Role:

We are seeking a highly motivated and compassionate Care Manager to join our team in the Social Assistance industry. As a Care Manager, you will be responsible for ensuring that our clients receive the highest quality care and support. Provide comprehensive care management services to clients, including assessment, planning, implementation, coordination, monitoring, and evaluation of their care plans. You will work closely with clients, families, and healthcare providers to ensure that all needs are met and that clients receive the best possible care. Care Managers are required to go into clients' homes and drive throughout Fairfield County.

Positions open:

  • Greenwich/Norwalk area
  • Bridgeport area

Starting Salary: 48,000-52,000 yearly


Minimum Qualifications:

  • Bachelor's degree in Social Services, Human Services, or related field
  • Ability to drive throughout Fairfield County
  • Valid Driver's License

Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop individualized care plans
  • Coordinate and monitor the delivery of services to clients, including medical, social, and community services
  • Provide education and support to clients and their families on available resources and services
  • Maintain accurate and up-to-date records of clients' care plans and progress
  • Collaborate with healthcare providers and community organizations to ensure that clients receive the best possible care

Skills:

As a Care Manager, you will use your strong communication and interpersonal skills to build relationships with clients, families, and healthcare providers. You will also use your knowledge of Medicaid and Long Term Care regulations to ensure that clients receive the appropriate services and support. Your patience and compassion will be essential in providing education and support to clients and their families. Additionally, your skills in discharge planning and case management will be critical in coordinating and monitoring the delivery of services to clients. Finally, your knowledge of community-based service practices will help ensure the safety and well-being of our clients.
