About Job
Social Service Professional to help older adults and persons with disabilities stay in their own home. Assess client’s needs, coordinate and authorize social services and work as part of a multi-disciplinary team to advocate and support client goals. Experience with biopsychosocial assessment, assessment documentation and care plan development. Ability to drive throughout Fairfield County and basic computer skills required. Travel reimbursement, health benefits and a flexible work environment.
Required Bachelor’s degree in, Social Work, Human Services or related field and one years of experience. Experienced assessors and bilingual English/Spanish are encouraged to apply.
