The Qualified Mental Health Professional works on the Intensive in-Home team as part of a three person team to provide interventions in the home and community setting with the identified youth and their family. The QP works with the families to plan, coordinate and monitor services through the provision of direct and indirect services to eligible consumers. Persons in this position report to the Intensive In-home Lead therapist and must participate in supervision with the other team members.

PRIMARY JOB RESPONSIBILITIES

  • Developing and monitoring the implementation of the person-centered plan in a variety of locations such as the recipient’s home, school, and other community locations
  • Monitors services delivery on a regular basis to ensure consistency and continuity with the person-centered plan
  • Working knowledge of the child mental health populations
  • Plan and engage in activities, provide supportive counseling and crisis management
  • Assist families in meeting educations, vocational, housing, and other basic needs
  • Enters service notes and other required documentation into information management system
  • Attend and participate in meetings on behalf of the consumer
  • Participate in weekly supervisory meetings with the team
  • Participate in clinical training events and any other meetings required

QUALIFICATIONS

  • Bachelor’s degree in a human service field and 2 years of experience with the mental health population (with at least 1 of those years of experience with children/adolescents)
  • Human service degrees include: social work, sociology, psychology or other human services

OR

  • A non-human service bachelor’s degree may be considered if they have 4 years of experience with the mental health population
  • Persons with a bachelor’s degree in a human service field and 1 year of experience with the mental health population can be considered for an Associates Professional position
