Position Summary: The Social Worker Lead supports the Director of Case Management with supervision, orientation, and mentoring of the social work staff. This position provides clinical assessment, therapeutic interventions, care coordination, transition planning and emotional support to patients and families at GRHC within their scope of practice. Ensures the integration of psychosocial support and care coordination across healthcare settings. Guides the team in delivering comprehensive, patient-centered care, managing complex cases, and fostering collaboration with internal and external healthcare partners. Develops and implements programs and initiatives to enhance service delivery and operational efficiency.

- Supervises and mentors case management staff, ensuring high standards of performance and professional development.
- Conducts performance evaluations, provides feedback, and supports staff in achieving their professional goals.
- Collaborates with Case Management Manager in hiring as well as the development of disciplinary and corrective action plans.
- Based on analysis of care team's performance, makes recommendations and role models techniques to improve their assessment skills
- Advances clinical skills in the development of Social Work interventions. Acts as a resource to team members.
- Ensures adherence to departmental policies, ethical standards, and regulatory requirements.
- Oversees and manages complex patient cases to ensure continuity of care and effective treatment plans.
- Coordinates with various providers to address patient needs, optimize treatment, and resolve barriers to success.
- Reviews patient histories and charts to determine appropriate referrals and develop individualized care plans.
- Develops and implements programs to enhance service delivery and patient outcomes.
- Monitors program effectiveness and ensures adherence to best practices and regulatory standards for staff's scope of practice.
- Conducts regular audits of case documentation and service delivery to ensure compliance and accuracy.
- Requires knowledge of family systems and interpersonal dynamics/relationships and is able to use that knowledge to assist patients and families in achieving therapeutic goals.
- Ensures patients and families receive appropriate education and resources related to medical, socioeconomic, and mental health support.
- Assists patients with transition of care planning, with a strong focus on Social Determinants of Health (SDoH) and linking patients to community resources.
- Advocates for patients' needs and collaborate with interdisciplinary teams to address any gaps in care.
- Builds and maintains relationships with external healthcare partners and community resources to enhance patient support.
- Designs and delivers training programs for staff to enhance their skills and knowledge.
- Maintains accurate records and generate reports on team performance, patient outcomes, and program metrics.
- Other duties as assigned.
Required Qualifications:
- Master's Degree in Social Work required from a school accredited by the Council on Social Work Education (CSWE).
- Current and valid State of Arizona license as a Social Worker required.
- 2 years of experience in Case Management or clinical social work required.
- CCM Certification preferred, must be obtained within 2 years of hire.
- Current and continuing CPR and AED training and certifications.
- AZ Department of Public Safety Fingerprint Clearance Card.
- State of AZ Driver's License with no significant violations during the last 5 years.
- Demonstrates ability to relate to diverse cultures and specifically the Gila River Community and/or other Native American cultures and community health services.
