We are looking for passionate and experienced professionals to join our team. We are currently hiring part-time/full-time Geriatric Care Managers to support older adults and their families through personalized consultation and care management services.

As a Care Manager you will:

Conduct regular home or facility visits to assess client needs (medical, social, functional and psychological).
Create and implement care plans tailored to each client's unique situation.
Coordinate services and care providers, and advocate for the client's best interest.
Communicate regularly with families and care partners to ensure high-quality, person-centered care.
Accompany clients to medical appointments and other activities as needed.
Work independently with ongoing support from a collaborative team.
