FFS - $90/per visit or $70.00/per hour for follow up/documentation

Part Time - $45k-50k - Mon - Wed 8am- 5pm; Wed - Fri 8 am-5pm or Fri-Sun 7-4pm

What Would You Do As a Licensed Social Worker

As a Licensed Social Worker at Your Choice At Home, you will play a crucial role in delivering direct social services within the home setting. You will work closely with patients and their families to assess psychosocial needs and develop a comprehensive plan of care that includes assessment findings, visit frequency, and established short- and long-term goals. Collaborating with an interdisciplinary care team, you will implement the therapeutic plan and monitor patient reactions to treatment, ensuring that any changes are documented and communicated effectively. Your ability to create goal-directed contracts with patients and their support systems will be essential in resolving medical social work needs, making a meaningful impact on the lives of those you serve.

What Matters Most

To succeed as a Licensed Social Worker at Your Choice At Home, candidates must possess a Master's degree in social work from an accredited institution and hold a valid NYS license (LMSW or LCSW). Relevant prior experience in a healthcare setting, particularly as part of a multidisciplinary team, is essential for effective collaboration and service delivery. Home care experience is preferred, as it equips you with the skills needed to navigate the unique challenges of providing in-home services. Strong communication and interpersonal skills are vital for building rapport with patients and their families. Additionally, having access to reliable transportation is necessary, as you will need to visit patients in their homes throughout Brooklyn and Queens.
