We are seeking a highly organized and detail-oriented Experienced Home Health Intake Coordinator to join our team. The Intake Coordinator plays a crucial role in managing the intake process for new patients, ensuring that all necessary documentation and information are collected and processed efficiently. This position requires a proactive approach to coordinating with patients, families, healthcare providers, and insurance companies to facilitate smooth transitions into home care services. The ideal candidate will have a strong understanding of the healthcare system and the ability to navigate the complexities of patient intake in a home health setting. In this role, you will be responsible for verifying patient information, obtaining necessary authorizations, and ensuring compliance with all regulatory requirements. You will work closely with various stakeholders to gather and process patient information, which includes medical histories, insurance details, and other relevant documentation. Your attention to detail and organizational skills will be essential in maintaining accurate records and ensuring that all patient needs are met promptly. The Intake Coordinator will also be involved in educating patients and their families about the home health services available to them, answering any questions they may have, and providing support throughout the intake process. This position requires excellent communication skills, as you will be the primary point of contact for patients and their families during this critical transition period. You will also collaborate with healthcare providers to ensure that all necessary information is communicated effectively and that patients receive the care they need in a timely manner.
