POSITION SUMMARY:
The Licensed Clinical Social Worker (LCSW) is responsible for providing expert-level clinical support and patient care, displaying advanced skills in completion of psychosocial assessments, develop treatment plans, and provides supportive/grief counseling, crisis intervention and group therapy, individual therapy, and family therapy. The LCSW functions as the hospice’s bereavement coordinator, playing a vital role in providing emotional, psychosocial, and grief support across the continuum and ensures the hospice program bereavement’s services meet regulatory compliance and patient/family needs.
POSITION EDUCATION/ QUALIFICATIONS :
- Valid license as a Licensed Clinical Social Worker to practice in the State of Texas
- Master’s Degree in social work, counseling or psychology from an accredited university
- Valid and current Driver’s License required
- Valid and current proof of Auto Insurance required
- Must be able to be sensitive to cultural and bilingual issues.
- Good written and verbal communication skills required.
- Ability to read, write and speak English
- Ability to communicate clearly and concisely with all levels of nursing, administration, and physicians
JOB KNOWLEDGE/EXPERIENCE :
- Minimum one ( 1 ) year experience working with child; adolescent, adult and geriatric/palliative care/hospice patients.
- Spanish language skills helpful
- Must demonstrate an understanding of community health, welfare, and social agencies
- Knowledge and/or strong experience with end-of-life issues
- Ability to work productively and effectively with minimal need for supervision.
- Ability to function well as a member of a multidisciplinary team
- Actively seeks opportunities for professional growth by attending educational seminars

Responsibilities:
POSITION RESPONSIBILITIES:
- Functions as the hospice Bereavement Coordinator
- Oversees the planning, implementation, and documentation of the hospice bereavement program in accordance with Medicare Conditions of Participation and state/federal guidelines, and following the organization’s policies and procedures
- Conducts bereavement risk assessments and coordinate ongoing support for families after the death of a patient
- Provides or coordinates individual counseling, grief support groups, memorial services and grief education resources
- Maintains bereavement records and ensures timely contacts per the bereavement plan of care
- Collaborates with hospice team members to identify families in need of high-risk bereavement support
- Provides staff education and community outreach related to grief and bereavement
- Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practices
- Provides direct social work services to patients and families, and/or groups
- Documents findings accurately and in a timely manner and formulate the plan of care based on findings
- Ability to represent the program well in interactions both inside and outside the hospital
- Conducts comprehensive psychosocial assessments of palliative care patients and their families
- Provides counseling, education and support to patients coping with chronic or life-limiting illness
- Collaborates with the interdisciplinary team (IDT) to support shared decision-making, advanced care planning, and resource coordination
- Assists families with navigating complex systems
- Serves as a liaison between patients, providers, and caregivers to address psychosocial barriers to care
- Participate in required in-services

B. Working Conditions : The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have low exposure to malodorous, infectious body fluids from patients and some minimal exposure to noxious smells from cleansing agents.
C. Occupational Exposure: This position has minimal to no exposure to blood, body fluids, or tissues and is an OSHA Category III (although situations can be imagined or hypothesized under which anyone, anywhere, might encounter potential exposure to body fluids). Person who performs these duties are not called upon as part of their employment to perform or assist in emergency medical care or first aid or to be potentially exposed in some other way.
