About Job

Keystone Hospice, located in Eagle, is expanding our amazing Social Work team. The ideal candidate is a collaborative problem solver with an analytical mindset to get the job done. Our Social Workers visit patients where they reside, which could include private homes, assisted living facilities, and skilled nursing facilities.

Our Social Workers utilize their end-of-life knowledge and skills to provide care to patients and their families. Regular bereavement support, daily interdisciplinary communication, and weekly sharing of positives and recognition are just a few examples of how our team supports one another.

Keystone is changing hospice care in Idaho with our exceptional clinical care, dedicated and specialized team, and unique holistic approach. As the only hospice owned and operated by a Veteran in the Northwest, we believe that success is achieved when we value our staff as our number one asset and provide greater access to care for our patients.
