As a key liaison between our agency, clients, caregivers, and clinicians, the Client Care Coordinator ensures seamless care delivery, outstanding communication, and ongoing satisfaction for both clients and staff. This is a temp-to-hire opportunity, with the potential to transition to a long-term role based on performance and fit.

Key Responsibilities:

  • Coordinate and schedule in-home visits, matching the right clinicians and caregivers to client needs based on skill set, availability, and location.

  • Serve as the first point of contact for clients and families - building strong relationships, answering questions, and resolving concerns promptly.

  • Maintain updated client records in the system, ensuring accuracy in scheduling, documentation, and service delivery details.

  • Monitor compliance with care plans, clinical documentation, and state/federal home health regulations.

  • Collaborate with clinical supervisors, HR, and field staff to ensure timely follow-up, onboarding, and quality care delivery.

  • Support business development by assisting with intakes, referrals, and client onboarding.

  • Participate in weekly team meetings to review client updates, staffing challenges, and service metrics.

Qualifications:

  • Prior experience in a home health, hospice, senior care, or healthcare office setting strongly preferred.

  • Strong scheduling, coordination, and multitasking skills.

  • Excellent communication and interpersonal abilities - both written and verbal.

  • Proficiency with EMR systems and Microsoft Office (Word, Excel, Outlook).

  • Detail-oriented, proactive, and compassionate mindset.

  • High school diploma or equivalent required; college coursework or degree in healthcare admin, human services, or business is a plus.
