Client Care Coordinator - Home Health (Temp-to-Hire)

As a key liaison between our agency, clients, caregivers, and clinicians, the Client Care Coordinator ensures seamless care delivery, outstanding communication, and ongoing satisfaction for both clients and staff. This is a temp-to-hire opportunity, with the potential to transition to a long-term role based on performance and fit.
Key Responsibilities:
Coordinate and schedule in-home visits, matching the right clinicians and caregivers to client needs based on skill set, availability, and location.
Serve as the first point of contact for clients and families - building strong relationships, answering questions, and resolving concerns promptly.
Maintain updated client records in the system, ensuring accuracy in scheduling, documentation, and service delivery details.
Monitor compliance with care plans, clinical documentation, and state/federal home health regulations.
Collaborate with clinical supervisors, HR, and field staff to ensure timely follow-up, onboarding, and quality care delivery.
Support business development by assisting with intakes, referrals, and client onboarding.
Participate in weekly team meetings to review client updates, staffing challenges, and service metrics.
Qualifications:
Prior experience in a home health, hospice, senior care, or healthcare office setting strongly preferred.
Strong scheduling, coordination, and multitasking skills.
Excellent communication and interpersonal abilities - both written and verbal.
Proficiency with EMR systems and Microsoft Office (Word, Excel, Outlook).
Detail-oriented, proactive, and compassionate mindset.
High school diploma or equivalent required; college coursework or degree in healthcare admin, human services, or business is a plus.
