Summary

We are seeking a dedicated Care Manager to join our team at the East Harlem Council for Human Services, Inc. In this role, you will play a vital part in coordinating care and support services for individuals in need, ensuring they receive comprehensive assistance tailored to their unique circumstances. Your work will directly contribute to our mission of enhancing the well-being of the East Harlem community.

RESPONSIBILITIES: Provide care coordination to the most vulnerable patients of the Boriken Neighborhood Health Center. Working within a patient centered medical home model, provide medical and psychosocial case management services, assistance with concrete service and entitlement needs, health education and wellness services. Coordinate referrals to services available through the East Harlem Council for Human Services, Inc. and other agencies. Actively participate in performance improvement and peer review process. Serve as an integral member of a multi-disciplinary care team.

DUTIES:

1. Provide care coordination along with a primary care team to assist patients of the East Harlem Council for Human Services, Inc. with chronic medical and mental health conditions and substance use disorders.

2. Complete a Comprehensive Psycho-Social Assessment for clients entering the Program.

3. Provide care coordination and education to patients diagnosed with substance use disorders and strongly encourage and advocate for services available through the East Harlem Council for Human Services, Inc. and other agencies.

4. Actively participate in staff, peer committees, including but not limited to meetings of the Social Services Department and Performance Improvement Committee.

5. Assist patients with social needs such as food insecurity, housing issues, legal needs, insurance or other health care coverage, or transportation and provide referrals and follow-ups, as needed (for example, helping patients fill out benefit applications or escorting them to community organizations to obtain needed services.

6. Use motivational interviewing, recovery and trauma-informed approaches when delivering services.

7. Identify the treatment services patients should receive based on severity of substance use diagnosis, stages of change, immediate needs using a person-centered approach

8. Collaborate with patient’s support network and other service providers on service coordination and case management support with the goal of patient’s recovery and stability in the community.

9. Assist patients with assessing healthcare and social systems, scheduling, arranging transportation and ensuring that patients are accompanied to their appointments if deemed necessary.

10. Develop and strengthen linkages between other institutions providing services to patients and their families.

11. Participate in internal and external meetings and outreach activities of the East Harlem Council for Human Services, Inc. and the Boriken Neighborhood Health Center.

12. Adhere to the Council’s policies and procedures and clinical protocols, and other standards of professional conduct. Participate in continuing education to maintain a solid knowledge base through in-service presentations at the Council and other agencies, and through other forms of continuing education.

13. Perform other functions necessary for the proper care of patients and functioning as deemed by the Council.

SUPERVISED BY: Director of Behavioral Health Services/LCSW

QUALIFICATIONS:

Bachelor’s in social work or related field. Five years’ experience providing supportive services to diverse populations preferred. Bilingual Spanish required. Experience working with patients and families, including individuals with HIV/AIDS, a history of substance use, mental illness, domestic violence and families at risk. Dedicated to providing high quality patient-centered services. COVID Vaccination required.
