Summary

We are seeking Care Managers to join our team at Park Slope Center for Mental Health. In this role, you will play a vital part in coordinating care for clients, helping them navigate their mental health journeys while ensuring they receive the necessary support and resources. Your contributions will be essential in fostering positive outcomes for those we serve.

Responsibilities

• Coordinate and manage care plans for clients, ensuring they receive appropriate services.
• Conduct assessments to identify client needs and develop individualized treatment plans.
• Utilize motivational interviewing techniques to engage clients in their care process.
• Provide crisis intervention and management during critical situations.
• Collaborate with multidisciplinary teams to enhance service delivery and client outcomes.
• Monitor client progress and adjust care plans as necessary to meet evolving needs.
• Facilitate access to community resources and support services for clients.

Requirements

• Bachelor's degree in social work, psychology, or a related field; Master's degree preferred.
• Experience in behavioral health or mental health settings is highly desirable.
• Strong crisis management skills with the ability to remain calm under pressure.
• Proficient in motivational interviewing techniques and client engagement strategies.
• Excellent communication and interpersonal skills for effective collaboration with clients and team members.
