About Job

**Job Overview:**

We are seeking a detail-oriented LVN, Intake Coordinator to join our healthcare team. The ideal candidate will be responsible for coordinating patient intake and care delivery, ensuring efficient and accurate service.

**Responsibilities:**

  • Receive and enter patient information in electronic system.
  • Verify insurance coverage and obtain necessary authorizations.
  • Maintain confidentiality of patient information in compliance with HIPPA regulations
  • Assist with medical records management including filing and organizing documents
  • Answer phone calls and respond to inquiries from patients and other healthcare professionals when necessary.
  • Provide administrative support.
  • Collaborate with the Administrator to ensure smooth transitions.
  • QA
  • Audit charts for quality of care rendered; i.e. orders followed, appropriate utilization of visits, appropriate contacts with physician and other disciplines. Will be rated by supervisor based on findings.
  • Assist in the field when necessary
  • Other duties as assigned, will be dicusssed during interview

**Expreience**

  • Prior experience in a healthcare setting, preferably with knowledge of Home Care Home Base and Epic software
  • Familiarity with various medical procedures and terminology
  • Strong communication skills to interact with patients and healthcare professionals
  • Ability to multitask in a fast-paced environment
  • Understanding of pain management protocols
  • Qualifications:
  • Must hold a current Licensed Vocational Nurse (LVN) license in the state of Texas
  • Knowledge of EMR (Hospice MD)
  • Strong knowledge of physiology and medical terminology
  • Experience in hospice, geriatrics, senior care, and occupational health is preferred

EXPERIENCE AND REQUIRED SKILLS:

  • 2 - 5 years' related experience; healthcare industry preferred.
  • Experience with Medicare, Social Security and Medicaid systems a plus.
  • Past patient interaction a plus.
  • Excellent written and communication skills.
  • A strong customer service philosophy.
  • Strong organizational and time management skills.
  • Ability to work independently.
  • Proficient with PCs and Microsoft Office applications.
  • Must be computer Savvy
  • Valid Texas Driver's License
