About Job

Make Lives Better, Build Your Future: Join The Heritage at Sagewood as our Resident Care Coordinator!

At Heritage Communities, you’re more than your title — you’re a vital part of a team that truly values every contribution. Whether you're just getting started or looking to grow in your career, we offer a workplace where you’re supported, appreciated, and empowered to thrive as you improve the lives of others in our senior living communities.

What You’ll Do:

As a Resident Care Coordinator, you’ll play a hands-on role in improving the lives of our residents by:

  • Leading and overseeing a team of caregivers, ensuring high-quality resident care and compliance with all regulations and company policies.
  • Developing and implementing bi-weekly staff schedules to ensure optimal coverage, efficiency, and minimal overtime.
  • Serving as a shift lead and on-call contact on a rotating basis, effectively identifying, investigating, and resolving operational issues.
  • Identifying staffing needs and assisting with the recruiting and selection process to maintain appropriate staffing levels.
  • Providing direct resident care, including assistance with daily activities, medication administration, and maintaining accurate resident records and charts.

What We’re Looking For:

  • 2+ years of experience as a certified caregiver and 2+ years of experience as a medication aide in a senior living or related industry.
  • 1+ year of experience in a leadership or supervisory role.
  • Must hold state-required caregiver (CNA) and medication aide (CMA) certifications with an acceptable record.
  • The ability to assist residents with limited mobility, lift/push/pull up to 50lbs.
  • Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions.
  • Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).

Why Choose Heritage Communities?

At Heritage Communities, with 17 locations across 4 states, you’ll find a place to grow your career and make a meaningful impact every day. We live by our core values — Trust, Respect, Compassion, Fun, and Always Learning — and we’re here to support you like a true team.
