About the Role:
At Corus Health, part of the CareM-managed network, we’re growing fast—and so is our need for caring, community-minded professionals. As a Community Care Coordinator based in Albuquerque, NM, you’ll be the trusted voice that helps patients understand their care options and take the next step toward better health at home.

Key Responsibilities:

  • Reach out to patients who’ve declined or delayed services to provide education and support
  • Make home visits or phone calls to those who are unreachable or uncertain about starting care
  • Partner with intake, sales, and clinical teams to ensure a seamless experience for each patient
  • Document outreach efforts, barriers identified, and outcomes in our EMR/CRM
  • Address non-clinical obstacles to care, such as transportation or caregiver hesitation
  • Assist patients and families with service agreements and admissions as needed
  • Build lasting relationships with patients, especially in underserved communities
  • Report trends and insights to help refine messaging and remove systemic barriers
Requirements:

What We’re Looking For:

  • High school diploma or equivalent required
  • Experience working with underserved or high-risk populations
  • Strong communication and interpersonal skills—you're empathetic, patient, and clear
  • Comfort working independently in the field with reliable transportation (mileage reimbursed)
  • Familiarity with documenting in EMR or CRM platforms
  • Bonus if you’re bilingual (Spanish-English preferred) or have outreach or public health experience
