Major Duties and Essential Functions

• facilitates patient's aftercare needs.
• communicates directly with providers about the acute and post-acute appointments and order needs.
• Schedules follow-up physician, labs, home health, nursing home placements, procedural, and financial business office appointments.
• Obtaining insurance authorizations, arranging transportation, completing forms, faxing, filing scanning, and documentation.
• Works under pressure and meet deadlines.
• Adapts to workload demands and sets priorities.
• Positive, professional presentation.
• Builds rapport quickly and develop working relationships.
• Works cooperatively with the program staff.

Minimum Qualifications

• High School Diploma required.
• Bachelor’s degree in social work preferred.
• Minimum of 3-5 years of experience in the behavioral health field.
• Knowledge of mental health resources in the community and how to access.
• Computer and electronic medical record (EMR) technical experience.
• Knowledge of or experience in mental health or chemical dependency fields.

• Salary Minimum $60,952.00
