DEPARTMENT: Oncology Services
Position Summary
The Oncology Social Worker is a key member of the cancer care team, providing compassionate counseling and supportive services to patients and families—primarily in the outpatient setting—throughout the cancer journey. This role also serves as a trusted consultant to physicians and clinical staff, offering expert guidance on psychosocial concerns, community resources, and financial support options.
Essential Duties
- Deliver emotional support and counseling to patients and families coping with a cancer diagnosis, based on referrals from patients, physicians, or staff
- Collaborate with the Cancer Committee and oncology physicians to identify and address psychosocial needs within the Sibley and affiliated outpatient practices
- Facilitate a general cancer support group (typically bi-monthly) and contribute to Cancer Center events focused on patient and family well-being
- Identify patients with complex discharge needs and coordinate post-treatment plans in collaboration with physicians, care teams, and families
- Lead or participate in multidisciplinary Patient Care Outcome Meetings to create individualized, comprehensive care plans for patients with complex needs
- Serve as a consultant for inpatient cases involving challenging discharge planning, offering recommendations for appropriate follow-up care and resources
- Document assessments, plans, and interventions in the electronic medical record
- Evaluate patient needs for grant-funded transportation and coordinate logistics with all involved parties
- Act as a liaison between patients, providers, and insurers to ensure continuity and quality of care after treatment
- Facilitate communication across healthcare teams to support seamless care transitions
- Refer patients with age-specific protective needs (e.g., long-term care ombudsman, adult protective services, legal conservatorship) to appropriate agencies
- Address challenges related to aging and loss of independence by connecting patients with community-based elder care resources
- Assist patients facing financial hardship by helping them access pro bono services, disability support, and insurance resources
- Flag cases involving questions about medical necessity or care appropriateness and refer to the Physician Advisor for further review
- Licensed Social Worker in the District of Columbia
- Master’s degree in Social Work from an accredited institution
- Minimum of 2–5 years of experience in a healthcare setting (oncology preferred)
- Current CPR certification
Manager, Center for Patient and Family Services
Salary Range: Minimum 37.02/hour - Maximum 61.07/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
