The Holloway Community Program at Milford is looking to hire a Social Worker II.

The Holloway Community Program at Milford is located at 701 N. DuPont Blvd in Milford, DE 19963. The Holloway Community Program at ChristianaCare, established in 1989, is the primary care provider for HIV/AIDS patients in Delaware, serving approximately 65% of HIV-positive individuals accessing medical services, making it the largest provider in the state. The Holloway Community Program offers primary HIV medical care, access to the Delaware AIDS Drug Assistance Program, medication adherence education, social-work assessment, case management, and health maintenance.

The Holloway Community Program employs 51 professionals, including board-certified doctors, internal-medicine doctors, OB/GYN providers, nurses, pharmacists, psychiatrists, and behavioral health consultants.


As the Social Worker II, you will join the William J. Holloway Community Program, assisting patients with HIV/AIDS by coordinating resources and addressing potential barriers to care. You will help patients adjust to personal and environmental difficulties that may predispose them to illness or interfere with medical care.


Work Hours:

  • Monday through Friday from 7:30am to 4pm


Job Duties / Responsibilities:

As the Social Worker II, you will be a member of the practice care team, focusing on addressing financial barriers to care, care coordination, and connecting to community resources to address social care needs. You must have current knowledge base of the HIV disease process, assisting in educational efforts and assessing patient needs. You will be conducting financial screening for Ryan White, ADAP, and ADAP co-pay services, assist patients in enrollment into Delaware Medicaid or private insurance, and apply for ChristianaCare Financial Assistance and HCC/SFL. You will evaluate and provide psychosocial support to patients and their families and assist them in accessing supportive services in the community. You will maintain current knowledge of resources available to assist other healthcare providers and HIV program staff in care coordination. Participate in departmental meetings, multidisciplinary team meetings, case reviews, and contribute to performance indicators for the program.

Education / Requirements:

  • Bachelor’s degree in social work or related field from an accredited college/university required.
  • Licensed BSW or MSW as required by the state of Delaware.
  • Master’s degree in social work (MSW) from an accredited graduate school of Social Work preferred.
  • Minimum of 2 years of related clinical experience required.
  • Demonstrated interest in HIV/AIDS patient management.
  • Bilingual language skills are a plus.

You should be able to relate to people like and different from themselves, maintain flexibility and professionalism, communicate effectively with physicians, staff, patients, families, and agencies, act as a role model, function as a member of an interdisciplinary team, understand medical terminology, organize and prioritize work assignments, and maintain timely documentation.
