We have an exciting opportunity for a Social Worker (LSW) to join Lend a Hand, part of our growing care management program! Our SWs provide complex care management in the primary care setting. Through sensitivity to social drivers of health, understanding of the role behavioral health challenges often play in a patient's overall health, and deep knowledge of socioeconomic safety net systems and resources, the Social Worker helps patients overcome barriers and make meaningful progress toward health-related goals.

Summary
The Social Worker – MHMG role provides professional social work services that support the goal of the Population Health program to deliver risk-based population management services to patients at all times during their tenure of care.

Required Minimum Knowledge, Skills, And Abilities (KSAs)
- Education: Bachelor's Degree in Social Work from an institution accredited by the Council on Social Work Education required.
- License/Certifications: Active license by the Maine State Board of Social Work required.
- Experience: Experience providing therapeutic services to adult and pediatric patients with chronic medical issues required. Experience in client advocacy and case management required.
