Overview
We are seeking an experienced and compassionate Care Manager to join our great team of Angels. We are well-known and trusted in our community, serving Snohomish County for 10+ years. Homecare is our passion and speciality!

The position requires a strong understanding of senior care such as: in-home care, assisted living, adult family home, rehab, etc. The Care Manager will supervise the caregivers, collaborate team members, and develop and maintain close relationships with clients and families. Prior caregiving and leadership experience is important and will allow for strong connection and guidance to serve individuals and families in need of homecare assistance.

Responsibilities:

  • Conduct phone and in-home assessments to identify client needs and develop personalized care plans.
  • Participate in matching caregivers to clients and prepare/adjust schedules accordingly.
  • Conduct in-home training and orientation for caregivers assigned to new clients.
  • Conduct ongoing care and service check-in activities such as: in-home quality assessments, supervisor visits, and surveys to ensure the caregivers are performing well and client needs are met.
  • Supervise caregivers to ensure adherence to compliance, company policies and best practices, and address any training or skill development needed.
  • Engage in motivational interviewing and be an expert match maker of caregivers and clients.
  • Offer resources and support to clients and employees as appropriate.
  • Maintain accurate records and ensure all state and company required activities are completed and timely.
  • Participate in marketing activities within Snohomish County.
  • Collaborate with healthcare providers, social services, and community organizations to coordinate care.
  • Be a great team player, willing and flexible to support other team members, priorities, and business needs.
  • Participant in various daily activities including taking calls, lead intakes, filling in for lunches/breaks/if team member out, etc.
  • Required to be part of the on-call rotation for after-hours support.

Experience and Desired Skills

  • A background in caregiving. Homecare is highly desirable.
  • Experience in supervising, mentoring, and training.
  • Strong communication skills, both verbal and written, are essential for effective interaction with clients and stakeholders.
  • Excellent organizational skills and able to multitask.
  • Maintain calm and professional manner in high pressure and urgent situations.
  • Flexible to client and business needs.
  • Familiarity with Veteran Affair‘s processes is a plus.
