Z Best Home Care is seeking an experienced and detail-oriented Home Care Coordinator to join our dedicated team. In this role, you will coordinate ongoing care to ensure a smooth and efficient process between patients and caregivers. The ideal candidate will possess exceptional communication skills, maintain a high level of confidentiality, and be committed to providing outstanding service in a supportive and dynamic environment.

Key Responsibilities:

Oversee and coordinate patients' and caregivers’ schedules.
Maintain professional decorum and confidentiality in all patient interactions.
Utilize strong problem-solving skills to address and resolve any issues that arise proactively.
Manage and update patient records and reports with precision and efficiency.
Deliver exceptional customer service, handling patient and caregiver inquiries and concerns with empathy and effectiveness.
Stay informed about changes in MLTC regulations and other relevant policies to ensure compliance and best practices.
Collaborate with internal teams and external partners to support optimal patient care and service delivery.
Stay updated with HHA exchange

Qualifications:

At least 1 year of experience in a coordination role within the home care industry.
Proven experience with MLTC care coordination.
Proficiency in Microsoft Word and Excel.
Excellent written and verbal communication skills.
Ability to work accurately and efficiently in a fast-paced, evolving environment.
