Your Role

The Resident Care Coordinator is responsible for overseeing the daily care of residents in both assisted living and memory care. This role ensures that care plans are implemented accurately, caregivers are properly supervised, and care delivery aligns with Title 22 and internal compliance standards. The RCC works closely with the Executive Director and the Interdisciplinary Team (IDT) to monitor resident well-being, train and mentor staff, and maintain operational readiness for regulatory reviews.

You will serve as the main point of support for caregivers, providing guidance on care delivery, infection control, and documentation accuracy. The RCC is responsible for ensuring timely completion of personal care tasks such as grooming, bathing, and mobility assistance, while also monitoring health indicators and reporting changes in condition. This role is hands-on and requires a balance of leadership, direct care oversight, and strong communication skills.

Key Responsibilities

The RCC ensures care plans are followed accurately, and all personal care services are provided to the highest standard. You will supervise caregivers through daily rounds and observations, provide coaching, and conduct performance evaluations to maintain compliance and quality of service. The position includes oversight of documentation in the electronic care system, including tasks, vitals, skin checks, fall prevention logs, and incident reports.

This role also involves managing caregiver scheduling for coverage continuity, supporting care meetings with the IDT, and addressing family or resident concerns promptly and professionally. The RCC is expected to maintain a safe, clean environment and ensure infection control practices are consistently applied. You will participate in ongoing training sessions and provide education for staff on care standards, emergency protocols, and best practices in resident services.

Requirements

Candidates must be at least 18 years old and demonstrate the ability to lead a care team effectively. Previous experience in assisted living or memory care is strongly preferred, along with knowledge of care planning, regulatory compliance, and staff training. The role requires strong organizational skills, attention to detail, and the ability to multitask in a fast-paced environment.

Employment requirements include a TB test and health screening (LIC 503), current First Aid and CPR certification, and criminal background clearance (LIC 508). Physical requirements include the ability to lift up to 40 pounds, assist with resident transfers, and stand for extended periods during shifts.
