Description of Position:

This position is responsible for all activities taking place in a specified part of the Assisted Living or Memory Care Area of a given Community including managing care of all residents, providing oversight of the staff regarding activities in the area, communication with medical professionals and families about the needs of residents, maintaining compliance with all regulatory standards and communicating on all appropriate matters.

Responsibilities of the Resident Care Coordinator:

Managing Care of Residents:

The Resident Care Coordinator is responsible for insuring that all residents are evaluated prior to residency, every quarter, or as needs warrant and to ensure that services are delivered. The evaluation will be presented to the Nurse or Social Worker for approval. The Resident Care Coordinator communicates with residents, family, and staff on an as needed basis to meet residents’ needs. Resident Care Coordinator participates in Care Conferences every quarter with residents and family members to discuss resident needs and appropriate service delivery.

Managing all staff in Specified Care Area:

The Resident Care Coordinator is responsible for screening, hiring, training, and supervising all caregiver staff and activity staff in the Specified Care Area. This includes managing poor performance, coaching to improve skills, directing staff to provide activities that correspond to care plans and appropriate documentation and compliance with all human resource guidelines. Assures that all staff maintain appropriate annual training to keep up with state requirements.

Oversees the development and implementation of the Specified Care Activity Program:

The Resident Care Coordinator will provide guidance for the staff in provision of Activity Programs.

Provides services within budgetary guidelines:

The Resident Care Coordinator is responsible for inventory control and direction to staff in maintaining budget.

Continuous Quality Improvement:

Resident Care Coordinator is responsible for oversight and compliance with LifeStyles CQI Program and takes steps to assure that services delivered are completed as designed. Routine audits of medication administration by staff, documentation, resident weights, and reassessments will be done by Resident Care Coordinator to assure compliance.

Maintaining Compliance with all local, state, and federal guidelines:

Resident Care Coordinator is responsible for all local, state, and federal rules and regulations as they apply to services delivered in the Specified Care area. This involves staff selection, training and oversight, resident care and documentation, and compliance with HIPAA, OHSA, Blood borne Pathogen Guidelines, and any other rule that applies to the work we do.

Safety Compliance:

The Resident Care Coordinator is responsible for insuring that they and any employees reporting to them work in a safe manner and that unsafe actions are to be corrected. Compliance with Safety Committee Standards is required. Should workplace injuries occur, the Department Heads and Directors are to assist the Executive Director when required with the investigation, return to work, and management of the injured worker communication with care provider and LifeStyles.

8. Team Effectiveness:

The Resident Care Coordinator is responsible for fostering teamwork with co-workers, showing initiative in overcoming obstacles, taking actions, which assist others beyond job responsibilities, and having a positive impact on co-workers, residents, and visitors.

Requirements:

Qualifications:

To perform this job successfully, they must have leadership through planning, directing, controlling, communicating, and coaching. This position requires strong written and verbal communication skills, strong personnel skills, and positive community relations skills to be successful. Excellent written, verbal, grammatical and punctuation skills. Must be self-motivated and can work independently and make decisions. Excellent organizational skills; detail-oriented. Pleasant, professional and personal manner.

Education and Experience:

  • LPN or RN License highly preferred.
  • Must have proven management experience and ability to manage staff.
  • Minimum three years of experience in assisted living/memory care.
  • Experience with Microsoft Word, Microsoft Excel.


Physical & Working Demands:

  • Uses PC, telephones, word processing and other standard office equipment.
  • Verbally able to articulate in person and over the phone.
  • Attending business-related functions.
  • The Resident Care Coordinator encounters stressful conditions dealing with residents, employees, and business activities.
  • Must be able to work well under pressure, resolve conflicts, prioritize tasks, and follow through with ideas.
  • Excellent verbal and written skills, interpersonal and mediation skills, ability to evaluate workers’ performance, strong working knowledge of employment laws and company regulations.
  • Must be able to work evenings and weekends occasionally and maintain a flexible work schedule.
  • Sitting, walking, bending, data entry, reading, written and verbal communication, and lifting up to 50 pounds.
  • Comply with state specific requirements.
