**Social Worker - Case Manager**

Location: Seattle, WA
Start Date: ASAP
Schedule: 5x8 Day Shifts (Monday–Friday)
Travelers: Locals accepted | First-time travelers NOT accepted
Experience Required: Minimum 1 year
Weekend/On-Call: No weekends, no on-call

Role Summary:

This role supports case management in a community clinic setting. Responsibilities include:

• Conducting initial and 180-day assessments for assigned participants
• Coordinating hospital and SNF discharges, ensuring safe and appropriate plans
• Collaborating with hospital discharge planners and communicating with the Providence PACE IDT (Interdisciplinary Team)
• Supporting continuity of care and follow-up for all assigned participants

Required Certifications:

• BLS (Basic Life Support)
• MSW (Master of Social Work)
• Plus one of the following Washington licenses:
• LICSW (Licensed Independent Clinical Social Worker)
• LSWAIC (Licensed Social Worker Associate – Independent Clinical)
• LASW (Licensed Advanced Social Worker)
