About Job

We are currently seeking a Rehabilitation Counselor to work in our Behavioral Health Services Division. To provide care and supervision of all assigned clients to ensure a safe, healthy, secure, and pleasant environment, which promotes growth and development. Facilitate client growth and development toward independence in self-care, daily living skills, socialization, and community living. Participate in the development of the individual service plans and related documentation.

Other Responsibilities but not all-inclusive:

  • Provide guidance and support to all clients; teach and reinforce skill development in the areas of nutrition, basic self-care, routine medical care, and follow-up, as developed through the joint planning sessions of the individual service plans.
  • Review and modify individual service plans to accommodate growth and development.
  • Ensure the overall safety of assigned individuals by maintaining a hazard-free environment and avoiding potentially dangerous situations.
  • Successful completion of medication training. Maintain and monitor medications in accordance with individuals’ self-medication plans, ensuring compliance with State and Agency medication administration regulations.
Qualifications:
  • A.A. degree in the social sciences or health care related field and one year of relevant experience in the MH/MR field; or any combination of experience, education, or training that would provide the level of knowledge, skill, and ability required.
  • Good communication skills, both oral and written.
  • Ability to work independently and as part of a team.

Scope:
  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop, and bend, which may include direct intervention with residents.
  • Possess manual dexterity and fine motor skills.
  • May be required to possess a valid driver's license and an acceptable driving record (contingent upon facility location and client and program needs).
  • May require crisis intervention in emergencies.
  • May be required to report to work during emergencies including inclement weather.
