Job Summary: Outpatient Biopsychosocial Interventions (OBI) for people with Intellectual/Developmental Disabilities (IDD) is hiring a Collaborative Care Case Manager (CCCM). As CCCM, you will work closely on a small team to complete a biopsychosocial assessment, and person centered recovery plan. You will provide skills training, complex case management, and intensive cross systems collaboration to help clients meet their goals, and improve their health. OBI operates a shared caseload and as CCCM you will be expected to be attend daily meetings to collaborate as a team on planning for client's mental health care.

Individuals served through OBI will have IDD and co-occurring mental health needs such as severe mental illnesses, substance use disorders and/or chronic health conditions. OBI staff operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile.

As a CCCM, you may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms/crises, provide support/training to the system of support, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. Although staff members in this position are not typically LPHA's, staff members in this position are working under the supervision of an LPHA who provides ongoing evaluation and oversight of all assessments, diagnoses and treatment. Additional job duties maybe as assigned to meet the needs of the program.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education:

Required:

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License:

Required:

  • Current and valid driver's license and a clean driving record.

Experience:

  • One year of experience working directly with persons with intellectual disability or other developmental disabilities.

Hours: Full-Time.

Workdays: Monday - Friday.


Workhours: 8:00 AM - 5:00 PM


Salary: $23.25 - $24.41 an hour based on education.
