About Job

Responsibilities:

  • Provide a minimum of 4 visits per month for minor, and 6 visits per month for adult clients.
  • Plan to visit clients at home, in the office, and the community settings.
  • Maintain a manageable caseload to ensure outcomes and practice requirements are met while taking into consideration the level of care of clients served
  • Ensure the completion of required Daily Living Activities (DLA-20) assessments every six months for all assigned Adult clients.
  • Complete required monthly summary submissions at the end of each month
  • Provide identified PRP rehabilitation and support services according to the consumer's rehabilitation plan
  • Quarterly review and track updates to the client’s Individualized Rehabilitation Plan.
  • Complete client encounter notes within a 24-hour time frame and according to the goals on the Individual Rehabilitation Plan
  • Participate in ongoing training and supervision
  • Work cooperatively with family and other involved professionals to coordinate services with other agencies and programs in the best interest of the client
  • Responsible for carrying out services and supports that meet the clients' medical and necessary mental health needs as reflected in the clients' individual rehabilitation plan (IRP).
  • Promote independence, freedom of choice, and overall wellness.
  • Assist the client with monitoring and obtaining access to all entitlement benefits for which they may be eligible. Monitor benefit status monthly.
  • Be available to clients in the event of a mental health crisis while on duty.

Requirements:

  • A Mental Health Professional in the State of Maryland (Board Approved) licensed in one of the following: LMSW, LGPC, an Associate Degree, Or someone who is currently certified by the Commission on Rehabilitation Counselor Certification or CPRP.
  • Experience working with both Adults and Minors, and 2 years of work experience in PRP.
  • Must successfully pass all background screening required by the state
  • Completed CPR/First Aid training
  • Possess a valid driver’s license in good standing
  • Reliable transportation and auto insurance that complies with the minimum state requirements.
  • Pay for Performance: (P4P) ($21.50 - $27 per completed client submission) DOE
  • Flexible Hours/ Evenings and Weekends available.
