The Intake Coordinator is responsible for managing the intake process for new home care patients and providing data entry support to the home care staffing and clinical coordination teams.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Receives and generates referrals from agency staff and community sources.
- Follows up with all potential patients and referrals regarding home care services.
- Explains home care services to patients, family members, caregivers, and other community members who are interested in receiving care. This may be done over the phone, through email, or in person.
- Answers non-clinical questions regarding patient rights and other home care regulations.
- Maintains accurate intake documentation for all new home care patients.
- Shares any relevant information provided by the prospective patient or caregiver with the Scheduling Coordinators and field staff, as needed, to support a more thorough and informed initial evaluation before the start of care.
- Enters new patient information into the Electronic Medical Record (EMR) system for scheduling, documentation, billing & payroll purposes.
- Maintains open communication with management and promptly provides updates on any relevant changes or new information as they arise.
- Provides data entry support for staffing and clinical coordinators and assists with special projects as needed.
- Adheres to all state, federal, and company regulations and policies related to home care services.
- Participates in On Call rotation as needed.
- Consistently delivers excellent customer service to both internal team members and external clients, always ensuring a positive and supportive experience.
- Performs all other duties as assigned.
QUALIFICATIONS EDUCATION and/or EXPERIENCE
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- High school diploma or General Education Degree (GED) required.
- Experience with Home Care intake process preferred.
- Strong communication skills, including face-to-face, telephone, and computer skills.
- Strength in attention to detail and organization and the ability to work independently.
- Must be able to interact successfully with patients, families, coworkers, and other agencies to present a positive and professional image.
- The ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.
- Bi-lingual preferred
Job Type: Full-time
Pay: $20.00 - $21.50 per hour
