Manhattan Psychiatric Center (MPC), an Office of Mental Health (OMH) Joint Commission accredited facility, is recruiting an Addiction Counselor to join their Transition to Home (THU) team. The THU team provides a multi-disciplinary approach to care for individuals who have a history of homelessness. The goals of the THU team are to develop personalized treatment plans for active rehabilitation and reintegration to the community.
As an Addiction Counselor on the THU Team, you are a vital member of a multi-disciplinary team focused on utilizing an intensive rehabilitation model to help develop skills for successful community living including:
· Conducting the substance abuse assessments to determine the risk for substance use and harmful behaviors
· Identifying treatment services individuals should receive based on assessments and recommending treatment objectives
· Providing individual mental health, relapse prevention, harm reduction, and substance abuse counseling based on motivational interviewing and cognitive behavioral responses
· Assessing needs and providing support to individuals related to problem-solving, wellness self-management, housing, income support, education and vocational training, social supports, employment, and primary care
· Reviewing, evaluating, and modifying the treatment plan based on individual’s behaviors and response to treatment
· Conducting Integrated Dual Disorder Treatment (IDDT) groups and self-help groups as needed
· Advocating for individuals to secure emergency detox and rehabilitation care, and other community supports that promote IDDT
· Evaluating IDDT services and expected outcomes

Minimum Qualifications:
Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement – Statewide (HELPS) program, if they meet the following minimum qualifications for:
Addiction Counselor 1:
· Registration as a Credentialed Alcoholism and Substance Abuse Counselor (CASAC), CASAC Level 2, CASAC-Advanced Counselor, or CASAC Master Counselor in New York State.
Addiction Counselor 2:
· Registration as a CASAC-Advanced Counselor or CASAC Master Counselor in New York State and one year of post-registration addictions counseling experience in a certified or licensed program.

This position is eligible for a Downstate Adjustment of $4,000.
