Location: Minneapolis

Pay: $18.00-21.00/hour

Shift availability:

  • Day shift: 7am -3pm
  • Evening shift: 3pm-11pm
  • Night shift: 11pm- 7am
  • Monday to Friday Every Other Weekend

Job Description

As a Community Support Specialist (CSS) at Affinity Care MN, you will play a vital role in supporting individuals with disabilities to achieve their personal goals and live as independently as possible. You will assist with daily living activities, provide companionship, and ensure a safe and supportive environment. This is a rewarding opportunity for compassionate and dedicated individuals who want to make a positive impact in their community.

Responsibilities

  • Provide direct care and support to individuals with disabilities, including assistance with personal care, meal preparation, and medication administration.
  • Support individuals in community activities, social events, and recreational outings.
  • Encourage and facilitate skill development and independence in areas such as personal hygiene, communication, and daily living skills.
  • Implement and follow individual care plans, behavior management plans, and daily routines.
  • Maintain accurate and timely documentation of services provided, including daily logs, progress notes, and incident reports.
  • Ensure a safe and clean environment for individuals, adhering to all health and safety protocols.
  • Foster positive relationships with individuals, families, and other team members.
  • Attend training sessions, team meetings, and other professional development opportunities as required.

Qualifications

  • High school diploma or GED required; additional education in social services, psychology, or a related field is a plus.
  • Previous experience as a DSP, caregiver, or in a similar role is preferred but not required. Training will be provided.
  • Must have a valid driver’s license and reliable transportation.
  • Ability to pass a background check.
  • Strong communication, interpersonal, and problem-solving skills.
  • Compassionate, patient, and dedicated to supporting individuals with disabilities.
  • Ability to lift up to 50 lbs and assist individuals as needed.
  • Possibility of moving light furniture from unit to unit moves
