The Foster Care Coordinator will utilize trauma-informed, evidence-based practices to support children and youth in foster care with complex and high needs. This vital field-based role serves those not currently enrolled in home and community-based waivers, awaiting services, or receiving care through a Certified Community Behavioral Health Clinic (CCBHC).
This position involves travel primarily in the Sedgwick, Harvey, Butler, and Sumner Counties of Kansas.
In this role, you will:
- Assess, plan, and implement tailored care strategies for children and youth, ensuring their needs for permanency and family-oriented placements are met.
- Identify, coordinate, and monitor referrals for psychosocial, behavioral, and medical services.
- Oversee care plans as a consistent point of contact for families and caregivers throughout the care continuum.
- Communicate necessary health information to all stakeholders to assure quality and prompt care delivery.
- Advocate for children and families to ensure their needs and preferences are represented within the healthcare team.
- Provide education on trauma and specific disorders to members and their caregivers.
- Collaborate with child welfare and juvenile justice systems for the benefit of the children and their families.
- Connect families with additional services, including educational advocacy and behavioral health support.
- Work closely with a master's level case manager on more complex cases, ensuring comprehensive care planning.

Required Qualifications:
- One of the following:
- Master's Degree in Social Work, Nursing, Psychology, or a related field.
- Unrestricted, independent licensure as a Registered Nurse.
- Bachelor's Degree in social work or related human services field.
- 2+ years of experience in a community health setting.
- 1+ years of experience with multi-system children.
- Proficiency in MS Word, Excel, and Outlook.
- Ability to travel to visit Medicaid members in various settings.
- Reliable transportation and a valid U.S. driver's license.
Preferred Qualifications:
- Licensed Social Worker or clinical degree.
- Experience with electronic charting systems.
- Capability in managing community resources.
- Experience with populations having complex medical or behavioral needs.

The salary for this role varies from $23.70 to $46.35 per hour. Compensation is influenced by local labor markets, education, experience, and certifications.
