PD Care Coordinator Specialist

General Summary: Responsible for the coordination and completion of patient shifts and visits, maintenance and upkeep of scheduling records and logbooks. This includes the accurate and timely communication of scheduling changes between office and field staff as well as ensuring adequate staff availability and compliance.

Essential Functions:

General responsibilities:

  • Promote/exemplify the Company mission, vision, and values at all times.
  • Answering office phone.

• Demonstrate professionalism and responsiveness with office staff, field staff, prospective clients, and others who may communicate with the agency. •
The input of information into computer software systems.

  • Maintains all employees’ files and ensures they are updated and organized.
  • Office and clerical activities as directed.
  • Participate in weekly meetings.
  • Escalate client, caregiver, or other concerns to supervisor as needed.
  • Any other duty requested to maintain the operations of the business, including caregiving duties. Scheduling responsibilities:
  • Responsible for coordinating all schedules for new and existing home care services.
  • Compliance with all state and federal regulations
  • Develop knowledge of individual employees’ availability and skill level to match them appropriately with clients.
  • Ensure caregivers have met all HR requirements and possess the necessary skills for the assigned client (s).
  • Ensure timely staffing and scheduling of shifts/visits for caregivers in their appropriate areas per physician orders, insurance authorization, care plans,
and/or client requests.
  • Prepare and communicate all pertinent information regarding the case and client care to each caregiver.
  • Keep client and caregiver calendars and information accurate and current.
  • Communicate effectively with clients, their families, team members, and other healthcare professionals regarding their schedules and care staff.
  • Maintain patient confidentiality and quality care.
  • Promote the Company's financial success by maintaining proper caregiver pay rates and limiting overtime hours.
  • Train caregivers on electronic visit documentation and ensure shifts/visits are completed and verified for payroll and billing daily.
  • On-call responsibilities, evenings, and/or weekends as needed.
Requirements:

Experience:

  • At least one (1) year experience in a general office environment.
  • Preferred, Health care experience.

Skills:

  • Ability to communicate verbally and in writing effectively.
  • Highly organized with a strong ability to manage different priorities.
  • Ability to work with others and use resources to problem solve.
  • Apply good judgment to fast-changing situations.
  • Strong customer service skills.
  • Work with a sense of urgency while maintaining a high level of detail.
  • Computer skills include but are limited to Microsoft Office programs and scheduling software.
  • Must read, write and comprehend English.

Education:

  • High school diploma or equivalent required.
  • Bachelor’s degree, Associate degree
  • Certified Nursing Assistant (CNA)/Licensed Practical Nurses (LPN) is highly preferred

Licensure/Certification:

  • Current driver’s license in good standing throughout employment.

Physical Requirements:

  • Prolonged sitting, standing, and walking are required.
  • Ability to handle stressful situations calmly and courteously at all times.
  • Requires working under some stressful conditions to meet deadlines and Company needs.
  • Ability to provide direct patient care to include assistance with ambulation, transferring

Environmental/Working Conditions:

  • Works primarily in an office environment.
  • Some exposure to unpleasant weather.

Machinery/Tools/Equipment Requirements:

  • Reliable transportation and auto liability insurance.
  • Computer and basic office equipment.
