Job Summary:

Providence Health and Services is seeking an experienced Licensed Clinical Social Worker (LCSW) for a community-based outpatient clinic role within the PACE (Program of All-Inclusive Care for the Elderly) program. This position emphasizes case management, care coordination, and discharge planning for a geriatric patient population.

Responsibilities:

• Conduct initial and 180-day biopsychosocial assessments for assigned panel participants.
• Actively participate in care coordination and discharge planning in collaboration with hospital discharge planners, skilled nursing facilities (SNFs), and the Providence interdisciplinary team (IDT).
• Coordinate services for patients being discharged from hospitals or SNFs to ensure a safe and appropriate return to the community.
• Maintain accurate documentation in EPIC to support continuity of care and care planning.
• Collaborate with interdisciplinary team members to support participant health goals and address psychosocial needs.
• Educate patients and families about available community-based services and benefits including Medicaid, Medicare, and the Agency on Aging programs.

Required Qualifications:

• Master’s degree in Social Work (MSW)
• Minimum 2 years of case management or social work experience, preferably in a community or acute care setting
• Active LCSW, LMSW, or MSW credential (LCSW preferred)
• Travel experience required; no first-time travelers accepted
• Experience with EPIC charting system
• BLS Certification
• Strong understanding of discharge planning, community resources, and care coordination

Preferred Experience:

• Experience in a community clinic, LTAC, or trauma-level hospital
• Familiarity with Protective Services, including Adult Protective Services, emergency detention, and guardianship processes
• Skilled in handling psychiatric disorders, dementia, and drug/alcohol treatment planning
• Competence in assessments such as Geriatric Depression Scale and Clock Drawing Dementia Screen
