We are seeking Case/Care Managers to fill multiple positions with our client, a well-established social services agency in NYC. This organization has been serving diverse communities since 1965, providing high-quality, fully bilingual health and human services. As a recognized Patient-Centered Medical Home, they are committed to delivering coordinated, compassionate care that improves health outcomes and promotes overall well-being. Bilingual HIGHLY PREFERRED. Bachelors degree peferred or HS/GED Diploma with two years experience in the healthcare field.

Working within a patient centered medical home model, provide medical and psychosocial case management services, assistance with concrete service and entitlement needs, health education and wellness services. Coordinate referrals foe services. Actively participate in performance improvement and peer review process. Serve as an integral member of a multi-disciplinary care team.

Roles, Responsibilities and Essential Duties

Provide care coordination along with a primary care team to assist patients with chronic medical and mental health conditions and substance use disorders.

2. Complete a Comprehensive Psycho-Social Assessment for clients entering the Program.

3. Provide care coordination and education to patients diagnosed with substance use disorders and strongly encourage and advocate for services available.

4. Actively participate in staff, peer committees, including but not limited to meetings of the Social Services Department and Performance Improvement Committee.

5. Assist patients with social needs such as food insecurity, housing issues, legal needs, insurance or other health care coverage, or transportation and provide referrals and follow-ups, as needed (for example, helping patients fill out benefit applications or escorting them to community organizations to obtain needed services.

6. Use motivational interviewing, recovery and trauma-informed approaches when delivering services.

7. Identify the treatment services patients should receive based on severity of substance use diagnosis, stages of change, immediate needs using a person-centered approach

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
