Job Summary

We are seeking a dedicated Registered Nurse to join our PACE program team in a dual-capacity role that combines intake assessment and care coordination responsibilities. This position involves conducting comprehensive in-home assessments to determine medical eligibility for the PACE program, while also coordinating and delivering ongoing medical care to enrolled participants. This field-based role requires regular travel to participant homes and healthcare facilities, working collaboratively with the Interdisciplinary Team (IDT) to ensure optimal health outcomes for our participants.

Required Qualifications

  • Current Registered Nurse (RN) license in good standing (Oklahoma preferred)
  • Current CPR Certification required
  • UCAT Certification preferred or must be obtained within 30 days of hire
  • Previous experience with in-home assessments (UCAT) or community nursing strongly preferred (ADvantage)
  • Experience in geriatric care, home health, or PACE model care is a plus
  • Knowledge of federal and state healthcare laws and regulations
