Hospice Experience desirable. HCHB experience helpful but not required.
Will travel within the designated territory of Washington, Ramsey, Dakota, and part of Hennepin Counties. Mileage reimbursed at $0.50/mile.
Support individuals, families and groups in a variety of settings such as schools, hospitals, detention facilities and treatment centers. Assist clients who may need help with both temporary and chronic problems such as homelessness, unemployment, domestic violence, and substance abuse. Prevent, assess, evaluate, formulate, and implement a plan of action based on client needs and strengths, and intervenes to address mental, social, emotional, behavioral, developmental, and addictive disorders, conditions and disabilities, and of the psychosocial aspects of illness and injury. Engage in the administration of tests and measures of psychosocial functioning, social work advocacy, case management, counseling, consultation, research, administration and management, and teaching. Communicate with others impacted, other agency staff and coordinates services with other agencies, on behalf of clients. Conduct individual, group and/or family therapy if needed. Participate in utilization review and staff rounds as a member of an interdisciplinary team. Coordinate implements and follows-up on all tasks related to the discharge of participants. Monitor and document client's activities and progress; ensures that client's case records are updated with this information. Provide emergency assessments in consultation with psychiatrist. Advanced degree in social work is a minimum requirement. Supervised clinical experience.
