We are currently seeking a Care Manager who will be responsible for overseeing patient care, facilitating communication between family members and healthcare providers, and supervising caregivers. This role involves coordinating and implementing services for our clients in accordance with our mission and care standards, while ensuring compliance with regulatory requirements and maintaining accurate client records.
Candidates should ideally hold an LPN, RN, CNA, or MSW license, possess at least two years of experience in home care or a related field, and have reliable transportation along with a valid driver’s license. Strong written and verbal communication skills are essential for effectively engaging with a diverse range of clients..

- Must possess impeccable follow up and attention to detail and be able to exercise initiative, problem solve, apply sound judgment.
- Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families.
